Group Visit Request Form

You may now book your group visit to Impression 5 online! Simply fill out the form below and click the "Submit" button on the bottom of the page. Please be sure to completely fill out all sections. If there is a conflict with your chosen times, our Reservations Office will contact you to arrange another time. When selecting a workshop, please keep in mind that each workshop session has a maximum of 30 students. If your group is larger than 30 students, we will book the appropriate number of workshops for your group, starting at your chosen time.
